To our clients, be sure to bring the following documents with you:

  • Proof of identification (photo ID)
  • Social Security cards or an Individual Taxpayer Identification Number (ITIN) assignment letter for you, your spouse and dependents
  • Proof of foreign status, if applying for an ITIN
  • Birth dates for you, your spouse and dependents on the tax return
  • Wage/earnings statements (Form W-2, W-2G, 1099-R, 1099-Misc) from all employers
  • Interest and dividend statements from banks (Forms 1099)
  • All Forms 1095, Health Insurance Statements
  • Health Insurance Exemption Certificate, if received
  • Proof of bank account routing and account numbers for direct deposit such as a blank check
  • To file taxes electronically on a married-filing-joint tax return, both spouses must be present to sign the required forms

Please join me for the Kids Are 1st “Family Safety and Health Festival”.  This resource fair will  feature health screenings, exhibits, distracted teen driving demonstrations, free car seats and free ID kits (with fingerprints and photograph), food, music and more.

Come learn about car safety by bringing your child’s car seat for a free safety check. You are encouraged to register in advance for car seat inspections.

Please join me and the Los Angeles Regional Reentry Partnership for a "Breaking Barriers" free legal clinic. Assistance will be provided for  criminal record expungement and felony reductions under Propositions 47 and 64.  Also, there will be free live scans for criminal record clearing. Do you or someone you know have any of the following on your record?

  • Shoplifting $950 or less;
  • Forgery in the amount of $950 or less;
  • Passing bad checks $950 or less;
  • Grand theft $950 or less;
  • Receiving stolen property $950 or less;
  • Possession of a controlled substance;
  • Marijuana sales; or
  • Marijuana possession

Shop LocalAs the Thanksgiving holiday approaches, I would like to extend my heartfelt thanks to everyone in Assembly District 59 for your continued support and vote of confidence in me. Communities like ours are the drivers for change.
